Citing a Medical Report

Use the following format when citing a patient medical record when writing papers or case reports. This citation is based on the 6th ed. APA rules for a technical report -- 7.03, p. 205. Section 6.28, p. 185 is also helpful with date information and Reference Examples 7-11, p. 200 also helps show date format.

How to Cite a Radiology Report -- Citation example 

Interpreting Physician (Year, Month date [Dictation Date]). Exam [italicized]. City: Facility.

Remember these APA rules related to the References pages:

 

  • Invert all authors' names; give surnames and initials for up to and including 7 authors (e.g. Author, A.A., Author, B.B., &  Author, C.C.). From APA 6th ed., 6.27, p. 184.

  • If the reference list includes different authors with the same surname and first initial, the authors' full first names may be given in brackets. From APA 6th ed., 6.27, p. 184.

  • For books and reports the rules state: Capitalize only the first word of the title and of the subtitle, if any, and any proper nouns; italicize the title. From APA 6th ed., 6.29, p. 185.

  • References page -- APA publishes references in a hanging indent format, meaning that the first line of each reference is set flush left and subsequent lines are indented. From APA 6th ed. 2.11, p. 37.
